Your school deans and/or administration has told you this. Your mother probably has, too. “Remember to thank your interviewers!”
But it’s not that simple, is it? How do you do it? What do you say? Who do you thank?
Here are my suggestions.
1. The Who: The basic rule is that a thank you message is fair game for anyone with whom you directly interacted for a duration of time and got to know you a bit during interview day. So if you want to thank the residents, that’s cool but not necessary. Interviewers, in my opinion, absolutely need to get thank you messages. If a particular individual was very helpful, it’s totally appropriate to thank them but it’s not a necessity.
1. The What: email versus card. I bought really cute thank you cards, only to discover that most people don’t provide addresses much anymore and mostly give out emails. I think email is the better option, if only to assure your thank you message gets to the right person and as fast as possible. However, for my home institution’s program, I gave cards because I stopped by the office to drop them off.
2. The Where: send to either the program’s office or directly to the individuals. Remember to get emails prior to your departure. It’s easy to make a pitstop at the office and ask on your interview day rather than trying to recall people’s names days later.
2. The When: I’d email as soon as possible, if only because my memory is not very long. But anywhere up to a week is just fine. You want to consider the question: how long would you remember someone you interviewed?
3. The Why: The goal of the thank you message is to convey appreciation but more importantly, to remind them again of who you are and how serious you are about the program.
4. The How: Keep it short, straightforward, and specific. My general template went kind of like this:
Dear (insert name),
Thank you for taking time out of your day to interview me on (date of interview). I especially enjoyed our conversation about (insert specific conversation topic or something memorable and why it was enjoyable).
(Insert name of residency program) was indeed (insert synonyms of adjectives that the program itself uses to describe itself). I particularly liked (insert a specific detail or aspect of the program) because (provide one brief, specific explanation).
After spending a day at (insert name of residency program), I can absolutely see myself at this program. The (insert phrases or adjectives the program used to describe it’s residents or prized aspects) align with what I believe a physician should be and how I hope to practice medicine. Thank you once again!

Note: if you are applying in the South or in touchy freely programs like family medicine, peds, or psych, a handwritten thank-you is still highly valued over email, even if it’s super short. If you ask for the office address, they’ll give it to you and you can send all thank you’d to the same place.
Also- I agree that it’s not necessary to send thank yous to every person you interact with. But if a resident took you to dinner or interviewed you and you had significant interaction with them, thank them. They have some say over your rank, too.
